Baylor Scott & White Institute for Rehabilitation

Physical Therapist

PRN

Home Health Covering  Collin County

Base Rate: $40.00/Hourly (Gross)

PT Start of Care: $150.00

PT Evaluations/Discharge: $85.00

Reg/routine visit: $70.00 (weekend is same)

Case Conference Rate: $40.00

Mileage:  per company policy

Select Medical complies with state and federal wage and hour laws. Compensation depends upon candidate’s years of experience and internal equity.

Responsibilities

We currently have an opening for a (PRN) PT working with our Home Health agency. 

Choose to be an integral part of a team that focuses on rehabilitation of patients in their home. The Physical Therapist will be responsible for both evaluation and treatment of their patient caseload. 

Qualifications

  • Current state licensure or eligibility for state licensure as a Physical Therapist required
  • Certified BLS or completion in first 90 days of employment required
  • valid state Driver’s License
  • home health experience
