SSM Health Day Institute is seeking a dynamic Full-Time Case Manager to join our amazing therapeutic rehabilitation team. This Case Manager will enhance the lives of patients by connecting with patients and their caregivers from pre-admission to post-discharge. This post acute setting services comprehensive outpatient rehabilitation services, including Physical, Occupational, Speech, and Recreational Therapy.
SSM Health Day Institute Program is a specialized outpatient rehabilitation program designed for individuals recovering from a traumatic injury or illness, who no longer require 24-hour nursing or acute rehabilitative care. The individual benefits from a comprehensive, multi-disciplinary approach to treatment that focuses on building skills and maximizing independence in daily activities at home, at work or in the community.

Shifts: Monday - Friday, 40 hours a week, 8:00am- 4:30pm
Under general direction monitors and assesses patient plan of care from pre-admission to post- discharge. As part of the treatment team assists patient/family in goal setting to maximize patient’s independence in both home and community. Interacts with payers to obtain necessary authorizations for initiating and continuing care throughout admission to the program.
Professional Healthcare Degree
One year rehab case management experience (preferred)
