Case Manager, Brain Injury
Shift Hours: Full-Time, Mon-Fri, 9am - 5pm

Responsibilities
Provides support to the Outpatient Cognitive Rehabilitation Program for the Brain Injury population. Work closely with families as a patient advocate, community resource and liaison for patient referrals. The case manager is part of an interdisciplinary team approach to help post-acute brain injury clients rebuild cognitive skills, restore physical and emotional strength and maximize independence.
Qualifications
Minimum Education & Experience (Including Licenses)
- BSN in Nursing or Master’s degree in Social Work required.
- Current and valid NJ state licensure as a RN, LSW, or LCSW required.
- Previous experience on case management or related field required.
- Previous experience in Brain Injury strongly preferred.
