Shift Requirements: Thursday Nights plus opportunity to pick up additional shifts

Responsibilities
Position Summary
A licensed practical/vocational nurse provides direct patient care and functions as a part of the nursing team under the direction of a Registered Nurse. The role and scope of function is determined by the degree to which professional judgment is required to perform a particular function or procedure. The delegation of tasks or functions to the LPN/LVN is the prerogative of the RN within the scope of the State Nurse Practice Act.

Specific Duties
- Participates in data gathering and history-taking as a component of the overall Interdisciplinary Assessment Admission that is performed by the RN.
- Assists the Registered Nurse in formulating the nursing plan of care, which includes goals and prescribed action for goal achievement, and carrying out the plan of care on a day to day basis.
- Performs nursing care to assist patients with activities of daily living, including bathing, hygiene, dressing, toileting, and transfers.
Minimum Education & Experience (Including Licenses)
- Current state licensure as LPN/LVN required.
- Certified BLS or completion in first 90 days of employment required.
- Previous experience preferred.

Transitional care allows patients to receive medically-supervised therapy by highly-trained physical, occupational and speech therapists in a comfortable, friendly setting. This facility is designed to provide medical, nursing and therapy services to patients with the express purpose of "transitioning" them back to their prior living arrangements when possible.
Patients admitted to Penn State Health Transitional Care typically stay less than two weeks, depending on the patient's progress and discharge destination. Each patient, on average, can expect to receive one to two hours of therapy daily. Each patient has an individually-designed care plan with input from a physician, nursing staff, therapy team and case manager.
