Case Management Coordinator
At Select Specialty Hospital, a division of Select Medical, our specialty hospitals care for chronically and critically ill or post-ICU patients who require extended hospital care. We are currently seeking a Case Management Coordinator to join our team.
The Case Management Coordinator is an administrative position with the goal of relieving Case Managers of time consuming tasks so the CM may focus on patient care. This position assures there is ongoing communication between the patient, their family and the clinical team within a positive cohesive team environment. The Case Management Coordinator will provide clerical and department assistance with the daily functions of the Case Management Department.
Welcomes newly admitted patients and their families to the hospital by reviewing what to expect the first few days, encouraging open flow of communication, setting Care Conference, educating on Patient Satisfaction Survey process.
Assumes responsibility for the completion of the following with the support of the Case Management staff: Daily Projected Discharge Calendar, Weekly Commercial Net Revenue Report, Monthly Charge Audit, UR Quarterly Report, QAPI, Medicare Average Length of Stay Calculator
Under the direction of the Case Manager, coordinates team processes for the intention of keeping track of the patient's clinical timeline including yet not limited to Interdisciplinary Team Conferences, the Functional Skills Safety Assessment, Discharge Conference to Home, Patient and Family education, and help to finalize details of the DC Plan.
Conducts follow-up contact and, where appropriate, documentation of DC Planning Referral outcomes, Non-Medicare UR reviews.
Researches difficult-to-find providers of medical services necessary for safe transitions of care.
Initiates and properly files medical record documentation
Schedules patient follow-up appointments and documents on discharge/transfer records.
Initiates DC Planning referrals
Creates and forwards medical record packets for medical information exchange of Non-Medicare UR and DC Planning Referrals under the direction of the Case Manager
- Bachelor’s degree and healthcare experience preferred
- Customer Service experience a must.
- Goal oriented with a sense of urgency and persistence.
- Must have the ability to prioritize and organize multiple cases at any given time
